Do not compare one Indian year with one US medical-school year. The US path usually adds an undergraduate degree before MD. Add the full stack on both sides.

A larger first installment is common. It can bundle tuition with registration, hostel deposit or university charges. Later years are not a copy of year 1.

NEET is mandatory for NRI and OCI MBBS seats in India. The fee is the last step, not the first.
A dollar total is wasted if the college listing will not support the pathway the family wants. Check the school, not the brochure adjective.

The cheapest door is still a regular government merit seat, when the student can actually win it. An NRI row is a different cheque. Do not plan 2027 as if every NRI will get the Tamil Nadu rupee fee.
Private state NRI examples around ₹22-26 lakh a year can undercut a US$200,000 deemed stack. They still need hostel, deposits and a vacant notified seat. Affordability is the complete course, not year 1.
Most states in the last published map said the same thing: regular government is subsidized; private NRI is college-specific; deemed goes through MCC. Repeating that 27 times does not help. Keep the named exceptions.
