A non-citizen who finishes medical school in the United States is not automatically an IMG. A US citizen who finishes MBBS in India is an IMG. ECFMG follows the school’s country.

A recognised degree is not a licence. The graduate still has to meet the training and registration rule of the country where they want to practise.
Finish the internship. Then follow Indian registration and the postgraduate rule that year.

The graduate is an IMG. Plan ECFMG, USMLE, residency and state licence as a second career, not a small add-on.
A US MD or DO still has to meet the live Indian rule for a foreign qualification.
The family likes MBBS in India and also wants a later US return to stay possible. They do not have to pick a country in Grade 10. They do have to map the US courses against NEET PCB, run a diagnostic before buying a one-year or two-year plan, start the passport and OCI file, price seats by type, and check ECFMG school notes before anyone falls in love with a campus name. This is a planning sketch, not a result.

India, if NEET and counselling open a seat after Grade 12. The usual US path waits for college. Starting sooner does not make India the better career.
A government seat often is. Private, deemed and NRI totals can be high once hostel, deposit and the full course are added. Compare the official payable figure, not a national average.
Yes. Qualify NEET. Then use the desk that owns the seat. OCI alone is not an NRI seat.
Yes. ECFMG follows the school’s country. A US citizen from an Indian college is an IMG. A non-citizen from a US medical school is not automatically an IMG.

Last published AAMC 2025 note: 43 MD schools reported taking them. Funding and visa rules stay school-specific. Build a real list before you apply.
A US MD or DO path lines up more cleanly. India can still make sense when cost, timing or the family situation pull that way. Treat the later IMG step as its own plan.
Pick the country of practice first. Then the ladder. Then the live fee.
